Use this form if you wish to type in your
information on the form using the computer. The candidate must fill
in the top portion of the form, print the form, and give it to the
reference writer to hand write, type with a typewriter, or have
the form e-mailed to the reference writer. If both the candidate
and the reference writer wishes to type on the form, the candidate
must save the form to their computer and then e-mail it as an attachment
to the reference writer. The reference writer will complete the
form, print it, sign it, and mail it to the address listed at the
bottom of the recommendation form.

Reader is available at no charge from Adobe
Systems' World-Wide Web Site. To use Acrobat Reader with your
web browser you need to first download Acrobat Reader from Adobe's
Web Site and install it on your system.
