General Circulation Policy
Who may borrow
FHSU Students with a
valid FHSU ID Card
FHSU Faculty/Adjunct, Staff with a
valid FHSU ID Card
FHSU Alumni (must have Alumni card)
Community patrons with a valid community
card or temporary library card

General Collection, Best
Sellers, Magnetic Media, Thesis (some), Juvenile Collection,
Kits (some), Curriculum, Documents
(some), Western Collection and Videos
Non-Circulating Materials Collection
Reference, Documents, Western
Collection, Periodicals, Ethnic
Note: Special Permission must be obtained before Non-Circulating
Items may be check out (see Special
Collections Checkout)
Special Collections Checkout
Patrons may obtain special permission
to checkout non-circulating items such as non-display journals,
Government Documents, Reference Materials, Western Reference,
etc. A Special Permission Checkout
form for Non-Circulating materials must be completed and signed
by a department librarian.
All Special Permission materials must
be checked out and returned to the Reserve Desk.
How many items may be borrowed
There is no limit on the number of
items that may be borrowed.
Responsibilities
By using a valid FHSU ID Card or Community
Card to borrow materials (or allowing another person
to borrow materials with their card) the cardholder assumes
the responsibility for the materials
and agrees to the policies of Forsyth Library.
The patron is responsible for any
overdue, damaged or lost materials charges.
Fines will not be canceled resulting
from failure to receive library mail due to not providing
a current mailing address.

Library Card Costs
FHSU ID Cards may be purchased by
FHSU Faculty/Staff/Students at the Card Center, 2nd floor
in the Memorial Union, at a cost of
$5.00.
Community Cards may be purchased at
the Circulation/Reserve Desk at a cost of $25.00 (no expiration
date.) Patron will need to provide their Drivers License or
Picture ID in order to obtain a card.
Temporary Library cards may be purchased
for students 18 years of age and under with signature
of their parents (parents must show their ID) or with a school
group with teacher signature. Cost
of the Temporary card is $4.00 (expiration date is the August
following their high school graduation.)
Lost ID Cards
Patrons are responsible for all items
charged to their cards. If your library card is lost or stolen
you will need to notify Forsyth Library
(Circulation Department) so a block can be placed on yourcard.
This will eliminate he
possibility of others charging materials on your card. When
your library card is found, it may
be brought to the Reserve Desk so it can be reactivated.
Fines and lost book charges will not
be canceled for failure to notify the library of a lost card.
Any ID found at Forsyth Library will
be placed at the Reserve Desk and the card holder will be
notified by mail. Student ID's will
be kept for up to 2 weeks and are then sent to the University
Card Center. Community card holders
will be notified and the card will be destroyed after 3 months
if no response is received.

Item Renewals
Materials may be renewed if not
previously requested by another patron.
Materials may be renewed in person,
by phone or by returning your overdue statement with request
to renew written on it (a fine may be accrued when renewed.)
Fines automatically begin to accumulate
when the loan period time is up. Fines cease to increase
once an item is renewed, checked
in or is automatically charged to "Lost" after
it is 50 days overdue. Patrons are
responsible for any fine accumulated prior to renewal of
item.

Holds
Holds may be placed on any item.
The person requesting the held material will be notified
as soon as the material is available.
Any item loaned for 30 days or more
is subject to recall after it has been charged out for more
than two (2) weeks, unless
the material is being used by Faculty for a class.
Returns
Book Drops are located on the North
side (exterior wall) of the library and at the exit desk
in the library.
****Failure
to return materials to the Exit Desk (general collection)
or Reserve materials to the Reserve
Desk may result in an overdue fine and/or the material(s)
being charged to lost
and an additional fine assessed.
****All
materials checked out by "Special Permission"
must be returned directly to the Reserve
Desk.
General Collection Overdue
Overdue fines are applicable to any
and every item that is checked out.
Patrons who receive an overdue notice
for material which they have returned or renewed should notify
the Circulation Office.
Library privileges may be suspended
until materials are returned and all fines and fees paid.
1st Notice - one day after due date
2nd Notice - eight days after due
date
Final Notice - 50 days after
due date (item will be charged to lost)
****Failure
to receive an Overdue Notice or Bill & Fine Notice does
not exempt the borrower from
charges.
General Collection Fines
$.25 per day that material is overdue
(per item)
Overdue fines begin to accumulate
at $2.00
All fines will remain on patrons record
until paid and a Hold may be placed on your Transcript and
Enrollment until charges are paid
(FHSU students.)
The fine amount is not tabulated by
the computer until the item is returned, renewed or automatically
charged to "Lost" after 50 days overdue.

Charges For Lost And Damaged Material
Patrons are responsible for all material
until it is returned.
Lost item fees are charged for material(s)
that have not been returned.
Damage fees will be charged for items
that are returned damaged based on replacement cost.
Materials are expected to be returned
in the same condition as when it is checked out. According
to Kansas Law, stealing or mutilating library property is
a misdemeanor.

Replacement
costs are subject to review and adjustment for particularly
expensive and rare materials
Replacement costs will be
forgiven when material is returned or replaced.
Placing a Locate on a Lost or Non-Returned item does not
guarantee that Library charges will be canceled.
Any questions regarding overdue items and fines should be
directed to the Circulation Office.
Reserve Collection Requirements
Only FHSU students/faculty
or staff may checkout items placed on Reserve by instructors.
Exceptions are Value
Line and Microfilm By-Pass Key that may be checked out with
a Drivers License (ID or Drivers License
will be kept as collateral until the Reserve item is returned.)
Reserve Items Load Period
Loan periods are 30 minute, 1 hour,
4 hour, 1 day, 1 week, 2 week, and 3 week. All materials are
marked accordingly.
Because loan periods vary, patrons
are responsible for knowing when the item is due and overdue
charges will be assessed.
If item is in high demand, only two
(2) items may be checked out at a time.
Items with one (1) day or more loan
period may leave the library.
Material(s) with a loan period less
than one (1) day, labeled "For Library Use Only",
"No Overnight Checkout",
or "R" (reference), Do Not leave the library.
Videos on Reserve for less than one
(1) day may leave the library only if written permission is
given by the instructor.
Item Renewals

be brought back to the Circulation/Reserve desk to be renewed.

Reserve Collection Overdues
Overdue fines are applicable to any
and every item that may be checked out.
Overdue statements are mailed the
first day after the material is due.
Failure to respond to the overdue
statement will result in the item being charged to Lost.
Library privileges may be suspended
until materials are returned and all Fines and Fees paid.

Patron Confidentialiy
Library patron records are confidential.
Library staff/students will not divulge to one patron what
another patron has checked out. This
policy is compliant with Kansas State Law. Library patron
and circulation records pertaining
to identifiable individuals are specifically exempted from
confidential infromation.
