Library Instruction Request
Form
Library Instruction is available to all FHSU faculty, staff, and students, as well as to schools in the surrounding area. Classes can be tailored to fit your needs, from a basic tour and overview of library resources, to subject or assignment-specific instruction. Each class is led by one of our Reference Librarians and can be held in the library or in your mediated classroom. Please schedule instruction at least one week in advance to ensure the availability of a lab and sufficient preparation time.
Complete the form below with as much information as possible about your class and instruction needs. When choosing a date for your session, consider that students are most receptive to library instruction when their need for library resources is immediate (i.e., they have an assignment and a deadline for completion).
Note: Your session is not reserved until you receive confirmation of the date by a librarian.
