There are two ways for you to return this form back to us.

1. Select the email form button located at the top of the form. This will allow you to either email it back to us directly or it will allow you to save the form on your computer and email it to us as an attachment.

2. Select the print form button located at the top of the form. This will allow you to print the form. You can then either bring it to our office, Sheridan Hall 208 or mail it to us: International Student Services Office, FHSU, 600 Park St., Hays, KS 67601
