This application is to be completed and returned to the Special Education Department by all students preparing to take a practicum course. However, before completing this application contact your advisor for permission to enroll in practicum. Specific courses should be satisfactorily completed before enrolling in any practicum.

Application deadlines for submitting this document are the following: August 1 (Fall Term); January 2 (Spring Term); and May 25 (Summer Term). Return application to:
