Policy Concerning Gifts, Prizes, Awards and Promotional Items Purchased
Purchase of gifts, prizes and awards must be approved in advance by Shana Meyer, Assistant Vice President for Student Affairs/Student Life slmeyer@fhsu.edu. After approval be sure to follow the University policy for submission of a plan detailing the required documentation including tax information collected from the recipient by way of a W9 or W8BEN (contact the business office for form).
