Certificate Request Form
Certificates will be issued to individuals only upon successful completion of all FHSU Department of Geosciences certificate program requirements and course work. All individuals meeting certificate requirements will need to complete the on-line print request form to recieve a printed certificate.
Successful course and project completions will be confirmed before certificates are approved or printed. Please note only courses listed on the certificate program and successfully completed with a grade of B or better (unless otherwise noted) will be considered for eligibility.
Certificate Request Process
Certificate requests will be reviewed for approval twice each semester. Approved certificates will be printed after the close of the semester in which the printed certificate was requested, or the semester in which the last relevant course is completed. Please allow 2-4 weeks for processing following the end of the semester in which the printed certificate is requested.
Once your certificate request is reviewed, approved, and printed you will receive an email from geosciences@fhsu.edu verifying your approval and confirming pick-up or delivery details. Please watch for this email following the end of the semester in which you requested your printed certificate.
Should your certificate request be declined, you will recieve an email upon review outlining the critera not met or requesting additional details to completed the request review.
