To submit a request for a Tiger ID Card, please click here.
To obtain a card, please visit the Tiger Card office at the FHSU Memorial Union, room 208 with the following:
- Proof of identity that must be a government issued photo ID including: a valid driver's license, state issued ID card, or passport
- $15.00 fee
The Tiger Card will be issued with an individual's full legal name including hyphenations and suffixes. No nicknames or abbreviations will be allowed.
If the name on your proof of identity does not match your name in the FHSU system, please fill out a Request Name Change Form under Personal Information in myFHSU worklet in Workday.
Your Tiger ID card will be delivered to you the following week on the Fort Hays Tech | North Central campus.
