Insulin Aid
Insulin Aid provides financial assistance to students who suffer from diabetes. The American Healthy Heart Association and Health and Wellness Services understand the financial burden this can place on students. Therefore, Insulin Aid is an award-based fund that helps FHSU students balance the financial stress of college and diabetes.
You must complete the following two steps to apply for the Insulin Aid program:
- Complete and submit the application form below. To help us better understand your situation, please be as thorough as possible.
- Contact your pharmacy to request an official printout of your current medications. You must then mail or hand-deliver the pharmacy printout to Health and Wellness Services and indicate that it is for your Insulin Aid application. Your application will not be considered complete until we receive your current medication printout.
Once your application is complete, a Health and Wellness Services medical professional will review your information and forward it to the Insulin Aid review committee. Health and Wellness Services will contact you about your application status in approximately two weeks.
