Patient Rights
- Every patient has the right to be treated in a professional, courteous, and caring manner without prejudice on any basis whatsoever.
- Every patient has the right to request a health care professional of either gender or one who uses a particular treatment approach.
- Every patient has the right to participate in decisions concerning care received. This includes the right to accurate information regarding all aspects of care, including costs, risks, benefits, and limitations of diagnostic and therapeutic modalities, and the right to assistance in preparing an advance directive.
- Every patient has the right to a second opinion or referral.
- Every patient has the right to privacy during medical care.
- Every patient has the right to confidentiality of all communications with staff and confidentiality of the medical record, as well as to approve or refuse the release of information to the fullest extent provided by law.
- Every patient has the right to refuse recommendations for medical care.
- Every patient has the right to refuse to participate in any research program.
Patient Responsibilities
- Every patient is responsible for arriving as scheduled for appointments and for notifying our office if cancellation of an appointment is necessary.
- Every patient is responsible for providing accurate information that could contribute to diagnosis and treatment.
- Every patient is responsible for either carrying out treatment and education recommendations or accepting responsibility for the outcome.
- Every patient is responsible for becoming informed, through available printed material and/or questions to our staff, about the scope of basic services, the cost of services, and the necessity for additional medical insurance.
- Every patient is responsible for actively seeking clarification of any aspect of participation in our services and programs (including cost) that is not understood.
- Every patient has the right and responsibility to bring forward suggestions or grievances about our services. This includes the right to receive a personal response.
